BLDE UGET 2016 Application form: The application form of BLDE UGET 2016 has been released on August 20, 2016. The BLDE UGET application form 2016 is available in the online mode only. BLDE UGET application form 2016 can be downloaded from the link given below on this page. The last date to fill applications is August 28, 2016. Candidates are advised to read the eligibility criteria before filing up the BLDE UGET application form 2016. Admissions to approximately 150 MBBS seats will be given based on NEET 2016 scores.
Application fee of BLDE UGET 2016 and payment Mode:
BLDE UGET 2016 Application fee is Rs.1000 which can be paid online or through Demand Draft.
Procedure for applying:
Existing applicants: Candidates who have already applied for BLDE-UGET-2016 are requested to update their NEET-2016 All India Rank (Overall Rank) & result details if Qualified in NEET- 2016, by using login procedure.
New applicant: Qualified candidates in NEET-2016 can apply through online application by entering all required information & online payment of fees Rs. 1000/- on or before 28-08-2016.
Online Payment Mode:
The application form can be paid through Online (SBI Collect): Candidates have to take the print of Online Payment Receipt and attach the same along with BLDE UGET 2016 Application form and also keep a copy for record and enter the ‘SBCollect Reference Number’ of 10 digit (in the Online application form at the space provided.
Online payment procedure:
Select payment type ‘Online’
‘State Bank Collect’ window will open, at the left side of the bottom Click on ‘Checkbox’ to ‘Accept Terms & Conditions’ and Click on Proceed
On the next screen Select Payment Category as ‘UGET-2016’ and complete all the fields (Name, Gender, Date of Birth, Mobile No.) with correct information and proceed as instructed and Click on Submit button. On the next Screen ‘Multi Option Payment System’ will be displayed along with the different bank charges. Select appropriate Mode of Payment and the same will proceed to ‘SBI Secure Payment Gateway’ fill all the required information and Click on pay button.
Take the printout of ‘Online Payment Receipt’ and enter the ‘SBCollect Reference Number’ in Online application form of BLDE UGET 2016 in the space provided
Offline Payment Mode:
If paid through DD: Enter the six digit DD number in the Online application form in the space provided. (Send the DD along with BLDE UGET application form 2016)
Enter the name of the bank from where the DD was purchased in the space provided.

Details of information to be furnished during filling up the BLDE UGET 2016 application form:
Personal Details: Candidates have to give information like their name, Gender, Parent’s Name, date of birth, Domicile by birth, nationality, etc.
Contact Details: Candidates have to provide their corresponding address, contact number, Email Id
Selection of Test Centre: Candidates have to select the Test Centre from the available exam centres
Academic Qualification: Candidates have to provide their Year of Passing Class 10 and 12 examinations, marks obtained in class 10 and 12. If appearing in class 12 they have to click 'Yes' in class 12 appearing tab
Upload Scan Images in BLDE UGET 2016 Application Form:
Photograph & Signature: The scanned images of Photograph and Signature should be in jpg format only and it should be less than 1 MB.
Class 10 marks Card: Upload the scan image of 10th class marks card issued by the board.
Class 12 marks Card: Upload the scan image of 12th class marks card issued by the board (note: if already passed the examination)
Caste / Category Certificate: Upload the scan image of SC / ST / Locomotor Disability (as per in information brochure) / OBC (Karnataka only i.e. Category 1, Category 2A, 2B, 3A, 3B) issued by the competent authority
Documents to send along with BLDE UGET 2016 application form:
Candidates must not send any original documents along with the BLUDE UGET 2016 application form
Affix recent color photograph in the space provided
Photographs should be of passport size, sharp and clear.
Person should not wear Goggle, Cap etc which will mask identity. Application with smudged / tampered or hazy photograph will be rejected
Photograph should not be stapled or pinned on the application form. Signature and attestation is not required on the photograph.
Incomplete / incorrect applications are liable to be rejected without any notice to the candidate.
Any application received after the last date shall not be accepted; whatever may be the cause
BLDE UGET 2016 application form is said to be complete only if it is enclosed with Online payment receipt / DD and after verification of the same submitted in person / speed post / courier
