Important highlights of DU PG Medical Application Form 2017:

  • Candidates who plan to apply for more than one course have to fill separate application form

  • After downloading the application form or obtaining it in person, candidates have to fill it in their own handwriting with Blue/Black pen

  • Erasing, cutting and overwriting in application form will lead to cancellation of application form

  • All the instructions mentioned in DU PG Medical 2017 Application form will be mandatory to follow

  • Candidates will have to write their names in the back of demand draft/banker’s cheque

  • The last date of submitting application form is March 30, 2017

  • Please note that the candidature is subject to the candidate’s fulfilling the minimum eligibility requirements prescribed for applying to the concerned programme of study. In case a candidate does not meet the minimum eligibility criteria prescribed for applying to the concerned programme, it is done at the candidate’s own risk and cost and if at any stage, it is found that the minimum eligibility requirements are not fulfilled, the admission if granted, shall be cancelled ipso facto. And the fees paid, if any, shall not be refunded in any case.

Check below for the list of required attested copies of documents to be enclosed along with the application form:

  • MBBS/BDS attempt certificate

  • Registration Certificate from Delhi Medical Council/ State Medical Council/ Medical Council of India/Dental Council of India

  • Certificate in support of having put in three years’ service: only in case of candidates seeking admission to MD (Community Health Administration) course

  • The compulsory rotating internship certificate

  • Detailed marks certificate of qualifying examinations: I, II & Final Professional examination of MBBS/BDS or year-wise detailed marks certificates of Master’s Degree/ Bachelor’s Degree, as required course-wise

  • 10th/ Higher Secondary Certificate for verification of date of birth

  • Candidates with disability certificate, if applicable

  • Non-creamy layer certificate (for the current financial year) and OBC caste certificate as per the Central list published by the National Commission for OBC (two copies)

  • Employer’s Certificate and a No Objection Certificate (NOC), if employed (as given in the application form)

  • Schedule Caste (SC)/ Schedule Tribe (ST)/OBC certificate, if applicable (two copies)

  • High School/ Higher Secondary Certificate for verification of date of birth

  • Certificate in support of educational qualification-MBBS/BDS Degree/ Master’s Degree/ Bachelor’s Degree – as per requirement of the course
