Goa CET Medical 2015 Application Form Procedure: Instructions for filling the Application Form
Candidate is required to fill-in all mandatory asked information which are listed below-
Marks scored in high school and intermediate examination
Date of birth
Application form registration number
Application form number
Candidate is required to paste his/her recent passport size photograph in the box provided for it.
Candidate is required to submit the completed application form along with Demand Draft of Rs. 1500/- payable in favourof “Director of Technical Education” at Panaji and all asked document by visiting the notified Application Reception Centre (ARC) as per centre opted by him/her. A candidate has to necessarily visit the opted ARC for submission of application form. No other mode of application form submission is acceptable.

Statement of marks/Certificate/provisional certificate of passing HSSC
School/college leaving certificate
Certificate of residence
Certificate for reserved category claimed
Income Certificate (For Tuition Fee Waiver)

Last date (without late fee) for submitting the completed Goa CET Medical 2015 Application Form by the DTE office is April 1, 2015. Last date for submitting the completed applications form with additional late fees of Rs. 1000/- is April 30, 2015. Candidate can obtain Photo Admit Card on the day of submission of application form.

Dear Banu, under Rights for persons with Disabilities under Benchmark disabilities, auditory disability should not exceed 40% to be eligible for PWD reservation in NEET. Now you haven't mentioned the disability percentage and so that it makes it difficult to guess. MCI has approximately 20 approved disability assessment centres which are listed in mcc.nic.in. You can get assessed at one of these centres. If the disability is more than 40% then you are not eligible for PWD reservation but will be considered for category reservation if applicable.
