It is absolutely necessary to pass all your subjects in 12th grade, whether it is your 5th or 6th subject or it is any Board of Examination. It does not matter if you have good marks in all your other subjects, it is required for you to get at least passing marks in all your subjects in order to receive your Pass Certificate. The board offered you a 6th subject for you to have an option for your bachelor's degree. You need to obtain at least passing marks in all your 6 subjects.
You can choose Physical Education as an additional subject along with your PCM and Computer Science combination in Class 11 , provided your school permits it . Check with your school authorities for specific guidelines and availability.

Definitely if you missed your 1 practical it will affect your marksheet. To be very honest ,I would say that it depends on school to school and your relation with the school teachers and principal. If everything is good then no problem will be there otherwise if you have some misunderstanding with the teachers they will show some rudeness to you that you will face in the result . You may request them for the same or do something under the table .

In the CUET exam , candidates have the flexibility to choose the medium for each subject separately . So, even if you have selected Hindi as your overall medium, you are allowed to take the Physical Education paper in English, as long as it's available in that language.

Admission into an Engineering college requires you to have Science (Physics, Chemistry, Math, Biology is not required) in your 11th and 12th grades. Seeing you had all these subjects in your school, you are eligible to appear for the JEE exam. You just make sure you scored 50%+ in your 12th grade, that is one of the eligibility criteria. They don't mind that Math was your 5th or 6th subject, they just need Math in your core papers.
Getting admission to a good Engineering college depends on the score you get in the JEE Mains and JEE Advance exams.

The registration for your subjects in 12th is completed in 11th class and you must stick by it. You are not allowed to change your subjects after registration.
So unfortunately you can not change your subject and have to study what you selected at the time of registration.
