If one could not get marked copy of the marks card, one can file affidavit for explanation of discrepancy of the mistake. Affidavit be signed by the notary public or gazette officer. Additional Documentation:

Submission of additional documentary evidence in the form of marriage certificate and other evidentiary material to substantiate the case.

This would make the name difference not cause much concern about your NEET application.

In the third week of March neet will open window for any correction, if some mistakes have their while filling the form.
