Hello student in this case you've only one option to fill the bsc nursing form for aiims from a different mobile number /email id as you said you've mobile number is allowing you to fill the from.

The main reason you are unable to submit the basic registration form can be that the deadline for basic registration will be over. If not, then probably there might be some issue with the website or the registration process. There will be contact numbers or email id's

While filling the application form you need to give the details as per 10th certificate.And at the time of verification process you can submit a affidavit stating that your surname is Mohammad written as MD.This is will not create any problem future if you submit proper documents.
Total aiims seat for UNRESERVED CATEGORY SEATS is 584 including seats in all the AIIMS. In AIIMS delhi there are 51 unreserved category seats.
