While filling the NEET Application form, one has to follow certain rules guided by National Testing Agency (NTA). It is Not mandatory to mention Name and date on Photograph. So you can either make it or not , there is no problem with that.  But you should carry passport

Yes if it is showing that your id is already registered it means that you have used that email for registration before and it is still in their record. So you might not remember the password, you can click forget password and it will come in your email id to
