For NEET you must have to score More then 50% in PCB overall for general and for SC/ST it's 4t%. As per your above mentioned score you have more then 50% in PCB so you are eligible for appearing in NEET and AIIMS as well. For AIIMS 50%

Yes, the birth certificate is one of the document needed for depositing to AIIMS which is necessary and it should be self attested copy of certificate from the board showing your date of birth (original).
For ex- 10th board result contains student's date of birth.
