You should ideally put your official name on the JIPMER application form. The official name is written on your Birth certificate or other identity docuements like class 10 Admit card,Aadhaar card, passport etc. Please do not put initials in the JIPMER application form if it is not included

Aggregate of PCB should be nore than 60%. individual subjects need not be nore than 60%.

Means lets say even if you get 50in one subject, 60 in other, 70 in another then too its allowed which makes your aggregate as 60%.

Minimum 50th percentile is qualifying criteria for general category and closing rank for general category is 150 as per 2016 trends in JIPMER.
