No, It will be based on both 11th and 12th standard according to previous year.

Yes, you can apply for Jipmer . But usually the dates for jipmer and ramachandra exams coincide . So choose wisely about what exam you want to attend . All the best :)
There is no any number of Attempts in Jipmer Entrance Exam even it has also removed for NEET Entrance Exam by CBSE.
