In the application form of AIIMS MBBS 2019, there is an option of Permanent address and Correspondence address. If your permanent address is of Jharkhand so you can fill that address in the Permanent Address field and in the Correspondence address field you can mention the other address.

When you fill up the basic registration form, you would require to fill up your ID proof details. If Aadhar card is the only ID proof you have, you can enter the Aadhar card number as it remains the same after your name is corrected.
