The Uttar Pradesh University of Medical Sciences (UPUMS) releases the application form for CPNET. The CPNET application form 2025 will be released in online mode on its official website upums.ac.in. Candidates will have to fill the application form of CPNET 2025 within the stipulated time period. Candidates will have to register themselves on the application portal of UPUMS by filling up some basic details and creating a password.

Aspirants should have a domicile certificate of Uttar Pradesh only. Students from other states are not eligible for CPNET 2025.
Students should have completed the 10 + 2 standard from a recognised board with Physics, Chemistry, Biology or Maths with English as a mandatory subject
The candidate’s age must be between 17 to 35 years as on December 31, 2025.

Here's a step-by-step guide to fill the application form of CPNET 2025.
Step 1 : Click on the “New Registration” tab to register himself/herself.
Step 2 : Candidate has to fill the below mentioned details on the page that appears

Step 3 : Create a password and confirm the same and after entering captcha, click on “Register”. Candidate receive the user ID and password on their registered mobile number and email ID
Step 4 : After completing the registration process, come to the homepage and login using the credentials received on the email ID and mobile number. After successful login, the candidate land on the application portal of CPNET 2025 where they need to fill the following details.

Step 5 : Upload the below mentioned documents in the appropriate format and size.

Step 6 : After successfully uploading the documents, click on the “Submit Application” button. The candidate are redirected to the payment page.
Step 7 : Select the payment mode and pay the required application fee as mentioned by UPUMS for CPNET 2025. The fee can be paid via Debit/credit card, UPI, e - wallet. After the successful payment, the candidate get an option to download the application form in PDF format. Download the application form for future reference. An email is sent to confirm the successful submission of the application form.
