The Atal Bihari Vajpayee Medical University, Uttar Pradesh has released the UP CPET application form 2025 on the official website. The UP CPET 2025 application form can be filled out from April 18 to May 24 in online mode only. Filling out the CPET 2025 application form is important as only the successfully registered candidates will be able to sit for the exam on June 18, 2025. Candidates wishing to appear in the UP CPET exam must fulfill the application form of UP CPET 2025. The UP CPET application process is a multi-step form that can only be filled once the payment is done. In order to fill the application form of CPET 2025, aspirants need to have the required eligibility criteria as specified by the authority. Candidates can check the CPET eligibility criteria 2025 below:
A candidate must be a citizen of India.
Aspirants must have completed the age of 17 years as of December 31, 2025. For B.Sc paramedical courses in SGPGIMS, Lucknow, the maximum age limit is 30 years, while for other affiliated institutes, there is no maximum age limit.
Candidates must have passed 12th with at least pass percentage of 45% (40% for reserved candidates) with Physics, Chemistry, Biology and have passed in English individually.
Students from National Institute of Open Schooling (NIOS) and other open schools recognized by central and state boards are also eligible.
The candidate should be medically fit. No candidate is granted admission until he/she is declared medically fit by the Medical Board constituted by the Government Medical College or the District CMO.
For Bachelor of Optometry and Bachelor in Audiology and Speech Language Pathology, candidates who passed 12th with a minimum pass percentage of 45% (40% for reserved candidates) in PCM (Physics, Chemistry and Mathematics) and passed in English individually are eligible.
For Bachelor in Audiology and Speech Language Pathology, candidates must have qualified Class 12 with at least pass percentage of 45% (40% for reserved candidates) in Physics, Chemistry and Computer Science/ Statistics/ Electronics/ Psychology and passed in English individually.

Eligible candidates have to visit the official website of ABVMU University, abvmuup.edu.in and click on the link for Common Paramedical Entrance Test (CPET) 2025. Click on the register for Paramedical Bachelor. Candidates have to fill the basic details to complete the UP CPET registration process:

After submitting the asked details, the registration number and password will be sent on the registered email ID and mobile number. However, candidates must note that the information submitted at this stage during registration cannot be changed or altered at any later stage.
After successful registration, applicants must log into the application portal by clicking on the login for Paramedical Bachelor. In this stage, candidates will be required to change the password after the first login. Without it, candidates will not be able to proceed further with the fee payment & form filling. After successful login, pay the CPET application fee online through debit card/ credit card/ net banking or UPI.

If a candidate forgets the registration ID number, click on “Forgot Registration ID Number” and enter the details asked. The unique registration ID number is sent to the registered mobile number via an SMS/email.
Once the payment is completed, students have to fill the CPET application form and submit the same within the specified date and time. Candidates have to follow the given below stages in order to submit the UP CPET 2025 application
Personal information: in this stage, students have to provide their personal information. In this stage of the application form, most of the form is pre-filled using the registration data filled by the applicant during the registration stage.
Address: Candidates have to fill their residential details, including address, state, city and pincode, correspondence address and other necessary details.
Academic details: The applicants have to submit the educational qualification details for each course. In case, an applicant is still awaiting 12th result, they can select “appearing”. However, it needs to be updated during the counselling process
Choosing UP CPET exam centres: Candidates have to provide their preferred test exam centres in order of preference. A total of 3 choices can be made by the applicants.
UP CPET photo, signature & verification sheet: In this stage, candidates need to upload photo, signature and a scanned copy of duly filled verification sheet.

Refund and form preview: Candidates are required to submit the bank account details to process the refund in case the failure arises. After submitting the bank account details and uploading the bank passbook or a copy of a cancelled cheque, a candidate will move to the “form preview section.
Final submission: Before the final submission, verify the filled in details carefully and click on the declaration button. Submit the UP CPt exam form 2025. After final submission of CPET application form 2025, a confirmation email is sent to the registered email address.
