INI CET Difficulty Level Analysis 2026: How hard will INI CET be? Nobody can answer that with certainty before the paper, because difficulty is a feeling that candidates report after the exam. What we can do is measure what makes a paper feel slow and hard, and count it in past papers. For the INI CET exam on 1 November 2026 (January 2027 session), though, no verified difficulty review can exist yet, because the test has not been held. The format is confirmed: a computer-based test of 200 multiple-choice questions in three hours, with admit cards released on 26 October 2026.

The closest reference is the previous session. Careers360’s INI CET 2026 question paper analysis rated the July 2026 session paper as moderate to moderately difficult. It leaned toward conceptual clarity and clinical reasoning. This article gives the expected INI CET November 2026 difficulty level, built from that review and the official pattern, and shows how to plan your attempt. This will be updated with the actual subject-wise analysis, good attempts and cut-off trends after the exam.
We cannot see how hard candidates found past papers. So we measured what takes time. We counted about 765 questions from four papers (July 2021, November 2021, May 2022 and a second 2022 paper) and flagged each question as time-heavy if it was image-based, a multi-statement question, or a match or arrange-in-order question. We also looked at clinical cases and long questions (25 words or more). The flags come from keywords in the questions, so the percentages are approximate.

Paper | Time-heavy (image, statement, match/arrange) | Time-heavy plus clinical cases | Long questions (25+ words) | Median words |
July 2021 | 32% | 45% | 38% | 18 |
November 2021 | 24% | 41% | 17% | 11 |
May 2022 | 34% | 46% | 32% | 15 |
Second 2022 paper | 38% | 48% | 28% | 15 |
All four | 32% | 45% | 29% | 14 |
Two things stand out. First, about one question in three was time-heavy, and with clinical cases added, close to half the paper needed more than a quick fact recall. Second, the paper with the highest time-heavy share (38%) was the latest in our set, and the lowest (24%) was November 2021. So difficulty has moved around.
We ranked subjects by the share of their questions that were time-heavy or clinical cases.
Subject | Questions in sample | Time-heavy or case-based | Expected feel |
Dermatology | 22 | 95% | Time-heavy |
Radiology | 20 | 75% | Time-heavy |
ENT | 17 | 65% | Time-heavy |
Anatomy | 44 | 61% | Time-heavy |
Paediatrics | 29 | 59% | Time-heavy |
Surgery | 65 | 58% | Time-heavy |
Forensic Medicine | 34 | 56% | Time-heavy |
Pathology | 62 | 55% | Time-heavy |
Orthopaedics | 22 | 55% | Time-heavy |
Medicine | 58 | 52% | Mixed |
Psychiatry | 15 | 47% | Mixed |
Microbiology | 46 | 39% | Mixed |
PSM | 48 | 38% | Mixed |
Ophthalmology | 29 | 38% | Mixed |
Obstetrics and Gynaecology | 74 | 35% | Mixed |
Anaesthesia | 17 | 29% | Quicker |
Pharmacology | 66 | 26% | Quicker |
Biochemistry | 47 | 23% | Quicker |
Physiology | 45 | 13% | Quicker |
Small subjects (under 25 questions) are rough. The useful point is in the big subjects. Surgery, Pathology and Anatomy are time-heavy, while Pharmacology, Biochemistry and Physiology are quicker, because their questions are short facts. Those three are where you can score marks fast and save time for the slower questions.
Each section gives 54 seconds per question. Here is a simple example, with an assumption you should replace with your own pace.
Assumption: a time-heavy question takes you 90 seconds.
If 38% of a 50-question section is time-heavy, that is about 19 questions, which take 28.5 minutes. The other 31 questions then have about 16.5 minutes, or roughly 32 seconds each. At the four-paper average of 32%, it is about 16 questions and about 37 seconds for each of the rest.
This is arithmetic, not data from any paper. But it shows the real challenge: the direct-fact questions must be answered in 30 to 40 seconds, or the slow ones will run you out of time.
The expected rating for the paper is moderate to difficult.
Factor | Effect on difficulty |
About a third of questions are time-heavy | Raises it |
Nearly half are time-heavy or case-based | Raises it |
Locked sections and −⅓ marking | Raises it |
Median question is only 14 words | Lowers it, because most questions are short |
Large quick-scoring subjects (Pharmacology, Biochemistry, Physiology) | Lowers it for prepared candidates |
The real difficulty will be known only from candidate feedback after the exam.
Attempt the quick subjects first. Revise Pharmacology, Biochemistry and Physiology facts until they are automatic.
Practise images and statements. Do timed sets of image questions and multi-statement questions.
Train the 54-second rhythm. Use the last 15 days mock test plan and the time management guide.
Skip and mark. Leave a slow question, finish the section, then return within the 45 minutes.
Guess with care. See the guessing and elimination strategy.

Frequently Asked Questions (FAQs)
We expect it to be moderate to difficult. This is our judgement from past papers, because the exam has not been held yet.
In our count of past papers, Dermatology, Radiology, ENT, Anatomy, Surgery and Pathology had the highest share of image-based, multi-statement and case-based questions.
Yes. Each correct answer gives +1, each wrong answer deducts ⅓ of a mark, and unanswered questions score zero.
