The computer-based INI CET has 200 MCQs to be solved in 3 hours, in four time-locked sections of 50 questions each, with +1 mark for a correct answer and −1/3 for a wrong one. Here you will find the expected difficulty level, subject-wise expected feel, section-wise time management and an attempt strategy. Use it to plan your final days with a clear head. These figures are estimates, not official data, because AIIMS does not release the question paper.

We looked at about 760 distinct questions from four past papers: July 2021, November 2021, May 2022 and a second 2022 paper. We classified each by subject and by style, using keywords in the question. This is approximate, so treat the percentages as rough.

Each section gives 45 minutes for 50 questions, which is 54 seconds per question. Once a section ends, you cannot return to it. So the paper tests speed as much as knowledge. A question you cannot solve in about a minute should be marked and left, because you will not get a second look after the section closes.
Style | Jul 2021 | Nov 2021 | May 2022 | 2nd 2022 paper | Average |
Clinical case or scenario | 23% | 20% | 22% | 18% | about 21% |
Image-based | 19% | 12% | 15% | 22% | about 17% |
Multi-statement (1, 2, 3, 4 combinations) | 13% | 11% | 19% | 14% | about 14% |
"Except", "false" or "not true" type | 9% | 7% | 15% | 15% | about 11% |
Match or arrange in sequence | 5% | 7% | 7% | 8% | about 7% |
The median question stem was only 11 to 18 words long. So most questions are short and direct, and about one in five is a clinical case. The papers did not look like long case-based exams. Image-based questions were highest (22%) in the second 2022 paper, and multi-statement questions peaked (19%) in May 2022.
Marks are our estimates from the share of questions each subject had. The style columns show what to expect inside each subject.
Subject | Rough marks of 200 | Image- based | Case- based | Expected feel |
Obstetrics and Gynaecology | 17–22 | 8% | 18% | Mixed: facts, doses and a few statement questions |
Pharmacology | 15–19 | 6% | 11% | Fact-based, mechanism and dose |
Surgery | 15–19 | 12% | 38% | Case-heavy |
Pathology | 14–18 | 23% | 26% | Image plus case |
Medicine | 13–17 | 14% | 36% | Case-heavy |
Microbiology | 11–14 | 24% | 11% | Images and facts |
PSM | 11–14 | 10% | 8% | Statements and facts |
Biochemistry | 11–14 | 11% | 6% | Short, factual |
Physiology | 10–13 | 7% | 0% | Short, conceptual |
Anatomy | 10–13 | 39% | 11% | Image-heavy |
In the smaller subjects, Radiology (65% image-based) and Dermatology (55% image-based and 73% case-based) were the most visual in our sample, but with only 20 to 22 questions each, the numbers are rough.
Our expected rating is moderate to difficult. This is a judgement, not a measured result. Here is why:
Time pressure. 54 seconds per question and a locked section make a moderately hard paper feel harder.
Negative marking. Wrong answers cost ⅓ mark, so careless guessing hurts.
Question mix. Close to a third of questions are image-based or multi-statement, and these take longer than direct facts.
Short stems. Most questions are short, which helps. If you know the fact, you can answer quickly.
Area | Expected difficulty | Reason |
Biochemistry, Physiology, Pharmacology | Easier to moderate | Short and fact-based, with fewer images |
Obstetrics and Gynaecology, PSM, Microbiology | Moderate | Mix of facts, statements and some images |
Medicine, Surgery, Pathology | Moderate to difficult | More case-based and image questions |
Anatomy, Radiology, Dermatology | Moderate to difficult | Image-heavy |
Practise images. Spend time on gross specimens, histology, radiology and clinical photographs.
Train for 54 seconds. Do timed sets of 50 questions in 45 minutes. The last 15 days mock test plan and the time management guide can help.
Skip fast, return only within the section. Mark hard questions and move on.
Guess with care. The table shows the average gain per question.
Options you can rule out | Average marks from a guess |
None (blind guess) | 0 |
One | about +0.11 |
Two | about +0.33 |
This is arithmetic from +1 and −⅓ marking, not an official strategy. See the INI CET guessing and elimination strategy.
Real difficulty. It will be known only after the exam, from candidate feedback.
Cutoffs and scores. We have no verified basis to estimate them. Check the official NI CET result after the exam.
Section order. The exam page says there are four sections but not whether they follow subject groups. Do not assume.
Surprises. A new paper can shift the subject mix or the share of image questions.

Frequently Asked Questions (FAQs)
We expect it to be moderate to difficult, mainly because of time pressure and negative marking. This is our judgement, and the exam has not been held yet.
In the four past papers we studied, about 17% were image-based and about 21% were clinical cases. These are rough counts, and the real paper may differ.
Yes. Each correct answer gives +1, each wrong answer deducts ⅓ of a mark, and unanswered questions score zero.
