The authority has released the UP NEET MDS application form 2024 for round 1 counselling. Eligible candidates can register and fill UP NEET MDS application form on or before July 15, 2024. Candidates who possess an MBBS/ MDS degree from a recognized institution and have passed NEET PG/ NEET MDS examination can able to fill UP PG medical application form. UP NEET PG application form 2024- Eligibility criteria
Candidates should verify that they meet the UP NEET PG eligibility criteria before filling out the application form to prevent any potential discrepancies. The basic eligibility requirements for UP NEET PG 2024 are provided below.
Medical graduates who have passed the NEET PG 2024 examination by securing minimum NEET PG cutoff are eligible to apply.
Candidates who have completed their one-year compulsory rotatory internship on or before June 30, 2024 can fill the application form.
Candidates who have graduated with an MBBS degree from government or private medical colleges in Uttar Pradesh are eligible for Government State Quota seats and Private Medical College seats.
Candidates who have graduated with an MBBS degree from Aligarh Muslim University or Banaras Hindu University are not eligible for Government State Quota seats. These candidates are only eligible for admission to private medical colleges.
Candidates who possess an MBBS degree from institutions outside Uttar Pradesh are only eligible for admission to private medical colleges.

Enter NEET PG/ NEET MDS roll number and email ID (used for registration on NBE website)

The Directorate of Medical Education and Training (DMET), Uttar Pradesh releases the UP NEET PG 2024 application form in an online mode. Only those aspirants who meet the specified criteria, as outlined earlier, are eligible to apply for UP NEET PG admission. Online registration for UP PG medical: Begin by registering online for UP PG medical admission at the official website, i.e. upneet.govt.in.
Generating login credentials: To login, candidates have to use their NEET PG roll number and email ID (entered during registration for NEET PG. Candidate’s data from the NBE’s website will be repopulated on the portal, which will be used for further counselling process.
Filling out details: On the application form page, candidates need to fill their domicile, category, sub category, In-service (PMHS) employment status, MBBS passes status, address and other details.
Payment of application fee: After entering all the details, candidates need to pay UP NEET PG application form fee. It must be noted that registration fee is non-registration.

Downloading the confirmation page: Once the registration form is submitted, a registration number is generated. Candidates will have to create a password for choice filling and locking. At the end a registration fee slip will be generated. Candidates need to download their registration form and registration fee slip, as these will be required at the time of admission.

It's  a good idea that you've already emailed the NEET PG officials regarding the mistake of uploading your temporary Dental Council of India (DCI) certificate while the DCI number you mentioned is the permanent one.
Discrepancies in application documents can lead to delays during verification. In the worst-case scenario, your application might be rejected.
Since you have already contacted the officials, wait for their response. They are the best source of information on how to rectify this issue and whether it will cause any problems.
In the meantime, have copies of both your temporary and permanent DCI certificates readily available. This will help if the officials request any further clarification.
If you don't hear back within a reasonable timeframe (around a week), consider sending a polite follow-up email to the NEET PG authorities.
