FMGE Subject-wise Preparation Strategy 2026: FMGE subject-wise preparation strategy 2026 starts with the marks, not the books. The Foreign Medical Graduate Examination (FMGE) has 300 one-mark questions across 19 subjects, no negative marking and a pass mark of 150. NBEMS now holds it three times a year, with the latest session on 31 October 2026. Subjects do not carry equal marks, and each has its own style of questions. This article gives subject-wise marks, a four-tier priority order, a preparation strategy, the struggles candidates report, and a free practice paper to download.

The FMGE is a computer-based test of 300 MCQs in two sittings of 2 hours 30 minutes each. Each sitting has three sections of 50 questions, and each section is locked after 50 minutes. There is no negative marking. The pass mark is 150 out of 300 for every category, with no separate cut-off for either sitting. The paper gives 100 marks to pre-clinical and para-clinical subjects and 200 marks to clinical subjects, as per the FMGE exam pattern.
About 12% to 13% of candidates passed in June 2026. NBEMS's own scrutiny, as reported, found that 35.74% of first-time candidates passed, against 3.29% of those with more than five attempts. The gap suggests that method matters more than extra hours of reading.

The marks below follow Careers360's FMGE subject-wise weightage. The tiers are a planning aid built from those marks.
Tier | Subjects (marks) | Total |
1 | Medicine 33, Surgery 32, OBG 30, Community Medicine 30 | 125 |
2 | Anatomy 17, Physiology 17, Biochemistry 17 | 51 |
3 | Paediatrics 15, Ophthalmology 15, ENT 15, Pathology 13, Microbiology 13, Pharmacology 13, Forensic Medicine 10 | 94 |
4 | Psychiatry, Dermatology, Radiotherapy, Anaesthesiology, Orthopaedics, Radiodiagnosis (5 each) | 30 |
Tiers 1 and 2 add up to 176 marks, but reaching 150 from them alone needs about 85% accuracy. The 124 marks in Tiers 3 and 4 are part of the pass, not extra.
Subject (marks) | How to prepare |
Medicine (33), Surgery (32) | Revise system by system: common conditions, investigations and guideline-based management. Practise long clinical stems and image-based items. |
OBG (30) | Cover antenatal care, labour and its complications, and gynaecology staging. Practise case and image questions. |
Community Medicine (30) | Focus on epidemiology, biostatistics, health indicators, the national immunisation schedule and national health programmes. This content is India-specific and may not have been taught abroad, so build tables and revise them daily. |
Anatomy, Physiology, Biochemistry (17 each) | Anatomy through diagrams, nerve and blood supply, and clinical correlations. Physiology through concepts and graphs. Biochemistry through pathway and enzyme tables. |
Pathology, Microbiology, Pharmacology (13 each) | Careers360's analysis of past memory-based papers points to inflammation, neoplasia and glomerulonephritis in Pathology, TB, HIV and malaria in Microbiology, and antimicrobials and anti-TB drugs in Pharmacology. |
Paediatrics, Ophthalmology, ENT (15 each) | These are compact subjects. Take one clean pass, then repeat previous year questions. |
Forensic Medicine (10) | Short and fact-based. Study it from notes that follow the Indian syllabus. |
Six 5-mark subjects (30 in all) | Each can be covered in a day or less. A January 2026 topper (233 marks) named Orthopaedics as the most challenging subject, so give it extra revision. |
The full topic list is in Careers360's most scoring topics of FMGE. Answer every question, because there is no negative marking and a locked section cannot be reopened.
Struggle | Details | Strategy |
Vast syllabus and too many resources | Careers360 topper interviews, including the June 2025 AIR 1 and a January 2026 topper (237 marks), name too many resources and too little revision as the biggest mistake. | Use one resource per subject, two passes, and a daily mistake log. |
Limited clinical exposure and a different syllabus | Many candidates get little patient contact abroad and study a curriculum that differs from India's. | Practise case-based MCQs and learn the India-specific content. |
Format changes | The June 2026 paper had 14 video-based questions, and candidates protested. An NBEMS expert committee gave no grace marks but recommended deferring such questions for at least a year. | Practise image-based questions and check every bulletin for pattern changes. |
Locked sections | Each 50-question section ends after 50 minutes, about one minute per question, and cannot be revisited. | Practise in 50-minute blocks. |
An unsettled schedule | NBEMS added a third session and moved the January 2027 date. | Plan by subject passes, not by calendar dates. |
Careers360's FMGE June 2026 memory-based question paper with detailed solutions is a free PDF. NBEMS does not release the official paper, so this one is rebuilt from candidate recall. Use it for the pattern and the subject mix, not as an official document.
