    What Makes a NEET PG Question Difficult

    Difficulty rarely comes from rare facts. It comes from format. A question can test something every MBBS student knows. It can still feel hard. Why? It may come as a long clinical case. It may be a negative question. It may need you to read an image correctly first.

    Recent NEET PG papers show this clearly. Clinical vignettes make up close to a third of the paper. Image-based questions make up close to a fifth. That is a large share of the exam. So treat difficult questions as a separate skill, not an occasional surprise.

    NEET PG Clinical Vignettes: Where Marks Go Missing

    A typical NEET PG vignette does not test one fact. It tests a chain of facts. Age, history, exam findings, one lab value: each detail rules out an option. Only the aspirant who reads the full stem, before looking at the options, gets it right. Here is a real pattern from a recent paper. A woman has breathlessness for months. She has an irregular pulse. She has a mid-diastolic murmur. She had rheumatic fever earlier. The question asks which statement is FALSE.

    One option says presystolic accentuation is a hallmark feature. This is the trap. That finding disappears once atrial fibrillation sets in. An aspirant may spot mitral stenosis at once. But if they skim past the word FALSE, they still lose the mark. The lesson is simple. The medicine is rarely the hard part. Holding four or five details in your head, while the clock runs, is the hard part. Building this skill is also why active-recall based revision matters more than passive reading at this stage.

    NEET PG Difficult Questions: Cross-Subject Questions

    NEET PG has moved toward questions that do not sit inside one subject. A Pharmacology question may lean on Medicine for its logic. A Surgery question on operative steps may quietly test Anatomy. Close to a fifth of recent papers are built this way. So an aspirant who has mastered Pharmacology and Medicine separately can still get stuck. The question sits exactly at the point where the two subjects meet, and that combination was never practised.

    This is why subject-wise, chapter-by-chapter revision is not enough on its own. It builds the base. But NEET PG is testing whether you can move between subjects inside a single question, not just recall each one in isolation.

    NEET PG Difficult Questions: Image-Based and Negative Questions

    Two formats deserve special attention. They quietly punish well-prepared aspirants.

    The first is the image-based question. This share of the paper keeps growing. You must correctly read the X-ray, the histopathology slide, or the clinical photo first. Only then can the medical reasoning begin. A wrong read of the image means a wrong answer, no matter how well you know the underlying disease.

    The second is the negatively framed question. It asks which option is FALSE, is NOT associated, or does NOT fit. This flips your normal instinct. Instead of hunting for the right answer, you must rule out three options that all sound correct. Both formats can be learned. But general subject revision alone does not fix them. They need practice in their own right, under the same negative or visual framing the real exam uses.

    How NEET PG Toppers Crack Difficult Questions

    Toppers are not simply the aspirants who know more. They have built a reflex. They read the entire stem before looking at the options. They spot the trap word early: FALSE, EXCEPT, NOT, MOST LIKELY. They eliminate options with confidence, instead of hunting for total certainty on each one.

    A. Millard-Gubler syndrome

    B. Foville syndrome (correct answer)

    C. Benedikt syndrome

    D. Wallenberg syndrome

    Explanation: Millard-Gubler and Foville syndromes look similar: both cause crossed weakness plus facial palsy. The difference is the paramedian pontine reticular formation (PPRF). It is involved in Foville syndrome, which causes the horizontal gaze palsy. Millard-Gubler involves only the abducens fascicle, giving an isolated lateral rectus palsy instead.

    2. Anatomy and Orthopaedics. A basketball player twists his ankle in forced eversion while landing. X-rays are normal. He has marked tenderness and swelling just below the medial malleolus. Which ligament is most likely injured?

    A. Anterior talofibular ligament

    B. Calcaneofibular ligament

    C. Deltoid ligament (correct answer)

    D. Posterior talofibular ligament

    Explanation: Most ankle sprains happen in inversion, which strains the lateral ligaments. This case is the opposite: an eversion injury. That single clue points to the medial-side deltoid ligament instead.

    3. Biochemistry. A healthy, exclusively breastfed neonate develops vomiting, lethargy, jaundice, and an enlarged liver on day 4 of life. Urine tests positive for a reducing substance, but not on a glucose oxidase strip. Which enzyme deficiency is most likely?

    A. Fructokinase

    B. Aldolase B

    C. Galactose-1-phosphate uridyltransferase (correct answer)

    D. Glucose-6-phosphatase

    Explanation: A non-glucose reducing sugar, appearing as soon as milk feeding starts, points to lactose splitting into glucose and galactose. This is classic galactosemia, caused by GALT enzyme deficiency, and it matches the day-4 timing.

    4. Biochemistry: Inborn Errors. A 45-year-old man has urine that turns dark on standing. He also has bluish-black discoloration of the ear and nose cartilage, and early arthritis of the spine and large joints. Which enzyme is deficient?

    A. Phenylalanine hydroxylase

    B. Homogentisate 1,2-dioxygenase (correct answer)

    C. Branched-chain alpha-ketoacid dehydrogenase

    D. Tyrosinase

    Explanation: This is alkaptonuria. Homogentisic acid builds up. It darkens on air exposure, which is why the urine darkens on standing. Over years, it deposits in connective tissue, causing the bluish-black cartilage pigmentation described here, called ochronosis.

    5. Biochemistry: Vitamins. A vegan patient has megaloblastic anaemia and growing difficulty walking. Examination shows loss of vibration and position sense, along with spasticity and extensor plantar responses. Which vitamin deficiency fits best?

    A. Folate

    B. Vitamin B12 (correct answer)

    C. Vitamin B6

    D. Thiamine

    Explanation: Folate and B12 deficiency cause the same blood picture. This question is built around the nerve findings that tell them apart. B12 deficiency also causes subacute combined degeneration of the spinal cord. Folate deficiency does not.

    Para-Clinical Subjects

    6. Pathology. Psammoma bodies are a classic finding in all of the following tumours EXCEPT:

    A. Papillary carcinoma of the thyroid

    B. Serous cystadenocarcinoma of the ovary

    C. Meningioma

    D. Medullary carcinoma of the thyroid (correct answer)

    Explanation: Medullary thyroid carcinoma comes from parafollicular C cells, not follicular epithelium. Its signature finding is calcitonin-derived amyloid stroma, not psammoma bodies.

    7. Pharmacology. A patient on long-term linezolid for a resistant infection also starts an SSRI. A few days later, he develops agitation, hyperreflexia, tremor, sweating, and low-grade fever. What is the most likely cause?

    A. Neuroleptic malignant syndrome

    B. Serotonin syndrome (correct answer)

    C. Malignant hyperthermia

    D. SSRI discontinuation syndrome

    Explanation: Linezolid is an antibiotic, but it also acts as a reversible MAO inhibitor. Combined with a serotonergic SSRI, it causes serotonin syndrome. The hyperreflexia here is the key clue: it rules out NMS, which instead causes rigidity and reduced reflexes.

    8. Forensic Medicine. Two unrelated autopsies both show bright cherry-red post-mortem lividity. One scene had a faulty room heater. The other had a faint bitter-almond smell on opening the stomach. Which clue best tells carbon monoxide poisoning apart from cyanide poisoning?

    A. Colour of lividity alone

    B. Presence of petechial haemorrhages

    C. Almond-like odour on opening the stomach or skull (correct answer)

    D. Degree of rigor mortis

    Explanation: Both CO and cyanide cause the same cherry-red lividity, so colour alone cannot separate them. The bitter-almond odour is specific to cyanide, though a genetically fixed share of examiners cannot smell it at all.

    9. Pharmacology: Cardiovascular. A patient on digoxin for heart failure also starts a thiazide diuretic. A few weeks later, he develops nausea, visual halos around lights, and ventricular ectopics. Which thiazide-caused electrolyte change most likely triggered this digoxin toxicity?

    A. Hypernatraemia

    B. Hypokalaemia (correct answer)

    C. Hypercalcaemia

    D. Hypomagnesaemia alone

    Explanation: Digoxin competes with potassium at the Na-K-ATPase pump. Thiazide-induced potassium loss reduces this competition. Digoxin then binds more strongly, and its toxicity increases, even without any change in the actual dose.

    10. Community Medicine: Epidemiology. A disease has a basic reproduction number (R0) of 5. What share of the population must be immune to reach herd immunity and stop sustained spread?

    A. 20 percent

    B. 50 percent

    C. 80 percent (correct answer)

    D. 100 percent

    Explanation: The herd immunity threshold formula is 1 minus 1/R0. For an R0 of 5, that gives 1 minus 0.2, or 80 percent. This is a calculation-based question. It trips up aspirants who know the concept but have not practised the formula.

    Clinical Subjects

    11. Internal Medicine. A patient's ECG shows a positive deflection at the J point, giving a camel-hump look. Core temperature is 31 degrees Celsius. What is this ECG finding called?

    A. Delta wave

    B. Osborn (J) wave (correct answer)

    C. U wave

    D. Epsilon wave

    Explanation: A positive J-point deflection is the Osborn wave. It is the classic marker of hypothermia, usually appearing below about 32 degrees Celsius, which matches the 31 degrees given here. This clue separates it from the similarly named delta and epsilon waves.

    12. General Surgery. A 62-year-old man has painless, progressive jaundice. His gallbladder is smoothly enlarged and non-tender, and can be felt in the right upper abdomen. What does this most strongly suggest?

    A. Gallstone blocking the common bile duct

    B. Acute cholecystitis

    C. Malignant obstruction of the distal bile duct or pancreatic head (correct answer)

    D. Chronic calculous cholecystitis

    Explanation: This is Courvoisier's law. A palpable, non-tender, distended gallbladder in a jaundiced patient is unlikely to be gallstones. Long-standing gallstone disease scars the gallbladder and stops it from distending smoothly, which makes malignant obstruction the more likely cause.

    13. Internal Medicine: Hepatology. A 17-year-old has tremor, slurred speech, golden-brown rings at the edge of the cornea, and mild liver enzyme rise. Serum ceruloplasmin is low. What is the inheritance pattern of this condition?

    A. Autosomal dominant

    B. Autosomal recessive (correct answer)

    C. X-linked recessive

    D. Mitochondrial inheritance

    Explanation: This is Wilson's disease, caused by an ATP7B gene mutation that impairs copper excretion through bile. It is inherited in an autosomal recessive pattern, a commonly tested feature that separates it from other inherited liver diseases.

    14. General Surgery: Abdomen. A patient with acute haemorrhagic pancreatitis develops bluish discoloration around the umbilicus, and separately, bluish discoloration over both flanks. What are these two signs called?

    A. Cullen sign (around the umbilicus) and Grey Turner sign (flanks) (correct answer)

    B. Grey Turner sign (around the umbilicus) and Cullen sign (flanks)

    C. Murphy sign (around the umbilicus) and Rovsing sign (flanks)

    Explanation: Both signs come from bleeding tracking through tissue planes in severe pancreatitis. But they are named by location: Cullen for around the umbilicus, Grey Turner for the flanks. NEET PG often swaps these two names as distractors.

    15. Dermatology. A middle-aged patient has flaccid, easily broken blisters on the trunk and inside the mouth. Gentle sideways pressure on normal-looking nearby skin makes it separate. What is this sign called, and in which major autoimmune blistering disease is it positive?

    A. Nikolsky sign, positive in pemphigus vulgaris (correct answer)

    B. Nikolsky sign, positive in bullous pemphigoid

    C. Auspitz sign, positive in pemphigus vulgaris

    D. Koebner phenomenon, positive in bullous pemphigoid

    Explanation: A positive Nikolsky sign points to a split inside the epidermis. It is classically positive in pemphigus vulgaris. Bullous pemphigoid instead has a deeper, subepidermal split, giving tense blisters and a negative Nikolsky sign. This is exactly the distinction being tested.
