NEET PG does not officially release its papers. So most previous year questions come from memory-based recall, put together by candidates soon after each exam. These are not word-for-word copies. But they capture the pattern, the traps, and the concepts the exam repeats year after year. Practising these tricky NEET PG questions helps you build the habit of reading a clinical base carefully before jumping to an answer. That single habit can save you many marks on exam day.

1. Ankle ligament injury (Anatomy, NEET PG 2025)
Q: A patient has ankle pain after a forceful eversion injury. Which ligament is damaged?
Answer: Deltoid ligament. Eversion stresses the medial side of the ankle, not the anterior talofibular ligament, which tears in inversion injuries.
2. High-altitude breathing (Physiology, NEET PG 2025)
Q: A trekker develops altitude sickness. What acid-base change is expected?
Answer: Respiratory alkalosis. Hypoxia drives hyperventilation, which blows off CO2 faster than the body can compensate.
3. Family history of jaundice (Pathology, NEET PG 2025)
Q: A 16-year-old boy has jaundice, splenomegaly, and a father with similar illness. MCHC is high. Diagnosis?
Answer: Hereditary spherocytosis. Spherocytes lose surface area, which raises MCHC — a rare finding most anaemias do not show.
4. Rice-water stools with garlic breath (Forensic Medicine, NEET PG 2025)
Q: A patient has abdominal pain, garlicky breath, and rice-water diarrhoea. Likely poison?
Answer: Arsenic. It mimics cholera closely, which is exactly why this question trips up students who jump to phosphorus.
5. Pulseless patient with VF (Anaesthesia, NEET PG 2024)
Q: A patient in ventricular fibrillation has no pulse. Next step?
Answer: Defibrillate, then start CPR immediately, then check pulse. Checking the pulse right after the shock wastes precious compression time.
6. Flat lesions near the anal canal (Dermatology, NEET PG 2024)
Q: A patient has flat perianal lesions, a body rash, and patchy hair loss. Diagnosis?
Answer: Secondary syphilis. These are condylomata lata, often confused with HPV-related genital warts, which look very different.
7. Fever with very high WBC count (Pathology, NEET PG 2024)
Q: A patient has fever, hepatosplenomegaly, and WBC over 50,000/mm3. Genetic abnormality?
Answer: t(9;22), the Philadelphia chromosome. This points to chronic myeloid leukaemia, not acute promyelocytic leukaemia.
8. Fever with petechial rash (Microbiology, NEET PG 2023)
Q: A patient has fever, low platelets, and a normal clotting time but prolonged bleeding time. Diagnosis?
Answer: Dengue. A normal clotting time rules out a coagulation disorder and points straight to a platelet problem.
9. Exercise-induced cramps (Biochemistry, NEET PG 2023)
Q: A boy gets muscle cramps on exercise, with low lactate rise after exertion. Diagnosis?
Answer: McArdle disease. Muscle cannot break down its own glycogen, so lactate fails to rise during activity.
10. Numbness at night (Orthopaedics, NEET PG 2023)
Q: A patient has nocturnal tingling in the lateral three fingers, relieved by hanging the arm off the bed. Best test?
Answer: Carpal tunnel syndrome, confirmed with Durkan's test, which is more sensitive than Phalen's or Tinel's test.
11. Sheathed microfilariae (Microbiology, NEET PG 2022)
Q: Microfilariae with a sheathed tail and two terminal nuclei are seen. Diagnosis?
Answer: Brugia malayi. Wuchereria bancrofti is also sheathed but lacks these terminal nuclei — a fine but important difference.
12. Diabetic eye disease (Ophthalmology, NEET PG 2022)
Q: A diabetic patient has preretinal haemorrhages and new vessels at the optic disc. Next step?
Answer: Pan-retinal photocoagulation. Focal and grid laser are used for macular oedema, not this high-risk proliferative stage.
13. Fever with hepatosplenomegaly in UP (Microbiology, NEET PG 2021)
Q: A patient from Uttar Pradesh has pancytopenia and macrophages full of kinetoplast-bearing organisms. Vector?
Answer: Sandfly. This is kala-azar, common in the Indo-Gangetic plain, spread by Phlebotomus sandflies.
14. Breathing trouble after thyroid surgery (ENT, NEET PG 2021)
Q: A patient has stridor and failed extubation after total thyroidectomy. Cause?
Answer: Bilateral recurrent laryngeal nerve injury. A one-sided injury causes hoarseness, not airway obstruction like this.
15. Lithium toxicity (Pharmacology, NEET PG 2020)
Q: A patient on lithium develops tremors after starting a thiazide diuretic. Why?
Answer: Thiazides increase lithium reabsorption in the kidney, since lithium follows sodium in the proximal tubule.

Read the full clinical scenario before looking at the options. Many tricky questions hide the key clue in a single word, like "family history" or "bilateral." Watch for red herrings, such as a family history of heart disease that does not actually rule out a drug. When two options look similar, ask what specific detail separates them, rather than picking the more familiar-sounding term. Most NEET PG questions today are framed as short clinical vignettes rather than one-line recall, so it also helps to practise dedicated NEET PG clinical scenario questions alongside this set.
Once you have gone through these tricky questions, test yourself under exam-like conditions with a full-length NEET PG mock test based on the latest pattern.
