The Directorate of Medical Education and Research (DMER), Uttar Pradesh releases the application form for UP NEET 2025 counselling on the official website, upneet.gov.in. As per the latest update, the authority has closed round 5 UP NEET counselling 2025 registration window for BDS course. Candidates could register for round 5 counselling till December 12, 2025 (2 PM).
For UP MBBS/BDS 2025 registrations, aspirants must be qualified for the NEET entrance exam. Based on the UP NEET application form 2025, medical students will be granted admission to 6428 MBBS and 2251 BDS seats offered by the medical and dental colleges of the state. UP NEET application form 2025 must be filled in five simple steps, which include UP NEET registration, uploading of documents, payment of fees, submission of form and printout of the application form. While filling out the application form, candidates will have to provide their personal, academic, contact, NEET 2025 and other asked details.

Before filling out the application form for UP NEET 2025 admissions, aspirants need to go through the eligibility criteria as mentioned by the state counselling authority. Aspirants can check some basic UP NEET eligibility conditions here.
Candidates should be 17 years old as of December 31, 2025
Aspirants must fill the application form to be eligible for UP NEET admission.
NEET UG applicants should be bonafide residents of the state. In the case of aspirants who have completed Class 10 and 12 from Uttar Pradesh are not required to submit their domicile certificate.
Candidates who have passed either their Class 10 or Class 12 from any school located in the state have to provide a general domicile certificate.
However, students have to furnish their original domicile certificate if they have not studied from a particular state.
For admission to UP NEET/BDS 2025 private colleges, any candidate from any state will be eligible for admission.
To complete Class 12, eligible candidates have to obtain at least an aggregate of 50% in Physics, Chemistry, Biology/Biotechnology and English. While the reserved category and general PWD aspirants must secure 40% and 45%, respectively.
Along with this NEET cutoff percentile and scores should be obtained to qualify for the entrance exam.

To register themselves, aspirants need to enter the following personal, academic, contact, NEET 2025 and other asked details. The application forms should be filled with utmost care as a few details in it cannot be edited.
All the details filled in the form should be the same as mentioned in the NEET application form, as it will be verified during the counselling and admission process. Upon successful submission of details, click on save and next for the completion of the remaining steps.

For the successful submission of UP NEET 2025 application fees, it is mandatory to pay the registration fees. For filling out the application, candidates have to pay the fees in online mode using credit/debit cards or net banking.
The last step in filling out the UP NEET application form 2025 is the final submission. After completion of all steps as mentioned, the student must cross-check the details filled in the application form for DME UP NEET 2025, as some fields cannot be edited. The preview of the application form will be displayed on the screen. After checking all the details, aspirants can click on the final 'submit’ button for the final submission of the UP NEET/BDS application form 2025. Download and take a printout of the confirmation page for future reference.
Candidates whose applications will be accepted by the authority will be included in UP NEET merit list, subject to fulfilling the eligibility conditions. Based on the ranks, aspirants will also be called for NEET UP counselling 2025 for admission to medical and dental colleges.
