The schedule for first round of counselling has also been published.The application form was released in the last week of December and the final date of registration was January 11, 2022. The application process includes online registration, filling of the application form, uploading of scanned documents, and Chandigarh MBBS application form fee payment. Chandigarh MBBS/BDS Application Form 2021
The Chandigarh NEET 2021 application form is available online mode. In the application form, aspirants will have to choose whether they’re applying for MBBS or BDS or for both the courses. If aspirants want to apply for both MBBS and BDS courses, they must select both the options in the form. It is to be noted that the Chandigarh MBBS 2021 application fee for each subject is different which has to be submitted before the last date which is January 11, 2022. And since Chandigarh MBBS 2021 merit list will be derived on the grounds of NEET 2021 score/rank, only those who have qualified in the NEET exam can only apply.
Chandigarh NEET 2021 Eligibility Criteria
Aspirants can check the basic eligibility criteria mentioned in the points below.
Candidate should be an Indian citizen.
Candidates must be at least 17 years as on December 31, 2021.
Aspirants must have passed 10+2 with Physics, Chemistry, Biology/Biotechnology, and English. Aspirants must pass each of these subjects individually.
The minimum qualifying marks in the above-mentioned subjects are 50% for General candidates, 40 for SC/ST/OBC candidates and 45% for PwD candidates.
An aspirant appearing/appeared in Class 12 or other equivalent qualifying examination in 2021 where the result is awaited can also apply. However, he/she shall not be eligible for the admission, if he/she does not pass the qualifying examination with the required percentage prior to the first round of counseling.
Admission to MBBS and BDS courses will be on the grounds of NEET 2021 merit/rank. Therefore, all the applicants will be required to qualify for NEET 2021.

Step 1: Registration
Aspirants are required to register themselves first in order to apply for Chandigarh MBBS/BDS admission 2021. To start the registration process, candidates are advised to enter the name, valid email ID, and phone number. They also need to select a unique password and confirm it. If the registration is successful, the registration ID will be generated which will be the default user ID.
Step 2: Filling the Chandigarh MBBS 2021 application form
Aspirants need to log in to the newly created account using their registration number and password. After login, they will be redirected to the Chandigarh MBBS application form 2021, in which they need to fill all the relevant sections like 10+2 marks, NEET 2021 rank, and scores.
Step 3: Uploading of the relevant documents
In the second step, candidates are required to upload the scanned passport size photograph and signature. The acceptable image format is JPG whereas the maximum file size must be 20 KB.

Step 4 - Chandigarh MBBS 2021 application fee
This is the final step of all the candidates (except NRI candidates) in which they need to pay the Chandigarh MBBS application fee in the online mode using the SBI collect option. It is to be noted that the application fee is category-specific, therefore, one needs to pay the fee according to their category.

What after the submission of the Chandigarh MBBS 2021 application form?
Those aspirants who will successfully submit the Chandigarh MBBS application form 2021 will be included in the merit list. The Chandigarh MBBS 2021 merit list will be released in the online mode with the names of the eligible candidates along with their state merit rank and other NEET details. On the basis of their state merit position, candidates will be called for the Chandigarh MBBS/BDS 2021 counselling session in offline mode. The counselling session will be conducted in two rounds, followed by a mop-up round, if seats remain vacant. While filling the application form, candidates will be required to fill their choice of colleges as well as courses. Ive passed ISC, Class 12th final exam with 52 percent in PCB, Ive passed in 5 out of 6 subjects and have got Pass certificate and have only failed in Maths, am I eligible for neet ug? Or failing in Maths makes me ineligible for Neet ug?

I think mathematics will be your additional subject as even without failing your overall class 12th status is passed.
As for NEET eligibility you must have PCB and English as your subjects in class 12th and you must have passed all the subjects both in practical and theory.
For NEET eligibility PCB aggregate marks must be 50% or more for Gen category students and 40% for OBC/SC/ST category.
As you said you have aggregate marks of 52% in PCB subject which makes you eligible for NEET even of you fail your additional subject .
