Age: Candidates must have attained a minimum 17 years of age as of December 31, 2025.
Domicile: Only Madhya Pradesh domicile candidates can fill the MP MBBS application form 2025.
Educational qualification: To apply for MP NEET admission, students must have passed class 12 (students appearing for class 12 final exams are also eligible).
Minimum marks required in class 12: Applicants must score at least 50% marks (general category) in class 12 to fill the application form of MP MBBS 2025.
Qualifying exam: For MP MBBS/BDS 2025 admission students need to qualify the NEET exam.

Step 1: Profile creation
Creating a profile is the first step in filling out the MP MBBS 2025 application form. Applicants must fill out MP MBBS/BDS application form 2025 in online mode.
Enter NEET 2025 roll number and password
Enter the secret keys of 12 digits, the captcha code, and click the 'create profile' option.
Note: The secret key is made up of the candidate's date of birth and NEET score. For example, if a candidate's DOB is 23/04/2000 and their NEET UG score is 58, the secret key is 200004230058.
Step 2: Registration
Candidates need to complete MP MBBS 2025 registration after creating their profiles. Aspirants must follow the below mentioned steps for MP NEET registration 2025.
Enter name, parents' names, date of birth, and gender
Fill in contact information, including your phone number, email address, and photo ID number
Submit bank account information and select the 'continue' option
A one-time password (OTP) will be sent to the registered mobile phone number
Enter the received OTP
Create a new password
Double-check the information and then click on the 'Verify and Save' button.
A pop-up notice appears on the screen indicating that the MP MBBS application form 2025 profile was successfully created.
Step 3: Filling of MP MBBS 2025 application form
In the MP NEET UG application form, candidates need to fill in the below-mentioned information. It must be noted that the authority does not consider applications with the incorrect information for MP NEET UG registration 2025.
Domicile: The MP MBBS 2025 application form asks candidates 'Are you a domicile of MP?'. If yes, candidates have to enter the reference number of their domicile certificate, date and place of issue, and designation of issuing officer.
Category details: Candidates must choose one of the following categories: UR, EWS, OBC, SC, or ST.
PWD details: In the MP NEET 2025 application form, candidates are asked, 'Are you physically handicapped as per the guidelines by NMC'.
Details for freedom fighters/Sainiks: In the MP MBBS/BDS 2025 application form, candidates who belong to the freedom fighter or Sainik class must select the 'yes' option.
Qualifying exam details: Candidates are asked to fill out the information about their class 12 examination, such as total scores, subjects studied, subject-wise marks achieved, and passing status.
Declaration: Read the declaration, accept it, and click on save data.
Step 4: Uploading of documents in the MP NEET application form

Step 6: MBBS application form 2025: Registration receipt printout
The registration receipt for MP NEET form is generated once the fee has been paid. The candidate's responsibility is to check all details mentioned in the MP MBBS 2025 registration receipt. Candidates should download and print multiple copies of their registration receipts to keep for future reference.
Candidates whose names are included in the allotment list, have to submit the original documents at the time of admission. Those who fail to produce the same are not eligible for MP MBBS admissions 2025. Candidates must submit the original document for the verification process.
