MP MBBS Application Form 2018 –The Directorate of Medical Education (DME), Madhya Pradesh released the MP MBBS 2018 application form on June 15, 2018. The application form of MP MBBS 2018 needed to be submitted with the relevant proforma and affidavits, all of which can be accessed from this page. The DME had released the MP MBBS 2018 application form in online mode only. Candidates must have compulsorily filled in the application form of MP MBBS 2018 to be eligible for admissions to Madhya Pradesh's Government and Private medical and dental colleges.

Candidates who have already submitted the application form had the option to make amendments to the form. The online correction window was a one-time facility and candidates were required to save the changes, otherwise, the changes have not been considered.

Only those candidates who have obtained the qualifying percentile and equivalent marks in the NEET UG 2018 examination are eligible for the state quota seats. The last date to fill the MP MBBS application form 2018 was June 25, 2018.
Candidates will be applying to a total of 1585 medical and 1361 dental seats in the government and private colleges through the MP MBBS 2018 application form. Eligible candidates will be granted admission based on their institute/course choice filled during online counselling, after the release of the state merit list, and their NEET 2018 exam scores.

MP MBBS BDS Application Form 2018 – Eligibility Criteria
Candidates filling up the MP MBBS BDS 2018 application form must be Indian citizens.
The candidates should at least be 17 years of age as on December 31, 2018, and not more than 25 years.
Candidates must be a domicile of the state of Madhya Pradesh in order to be eligible to fill up the application form of MP MBBS BDS 2018.
Candidates who are wards of freedom fighters, defense personnel/All India Government service employees serving in Madhya Pradesh, need not fulfill the domicile criteria for MP MBBS admission 2018.
The candidates must have passed the Class 12 board exam with the subjects of Physics, Chemistry, Biology/ Biotechnology and English
The candidates are also required to score a minimum of 50% marks aggregate (for unreserved category), 40% (for reserved category), 45% (for unreserved Physically handicapped category) in the subjects mentioned.
Candidates must also score the required minimum percentile in the NEET 2018 UG examination to fill up the MP MBBS BDS application form 2018.

How to fill MP MBBS BDS Application Form 2018?
Step 1 – Registration
Candidates need to visit the official website of MP MBBS BDS 2018 registrations through the link given above and create their profile. Alternatively, they can visit the login page of MP MBBS BDS admissions and click on the ‘Create Profile’ button in the 'Candidate Corner'. The candidates need to enter their NEET 2018 roll number (or NEET 2018 Testing ID). This followed by entering of the secret key which is a combination of the candidates’ date of birth in the ‘dd/mm/yyyy’ format, year of passing class 12 in the ‘yyyy’ format and NEET 2018 score. The candidates are then required to click on the ‘Register’ button of the MP MBBS application form 2018.
Step 2 – Profile Creation
After the step of the registration, the details of the candidate will be published on the screen of MP MBBS BDS 2018 application form. Candidates are then required to enter their bank details such as account holder’s name, account number, IFSC code, and branch name. A valid mobile number and email ID have to be submitted as well. The candidates will receive a One Time Password (OTP) on their registered mobile numbers. Candidates are then required to generate a new password which will complete the process of profile creation of MP MBBS BDS application form 2018.
Step 3 – Submission of Academic and Personal Details
In this step, candidates are required to click on the ‘Registration’ button of the newly created profile of MP MBBS BDS 2018 application form. Candidates then need to upload details such as their category, Class 12 marks and other details as required.
Step 4 – Uploading of Documents
The documents such as NEET admit card, Class 10 and 12 certificates, domicile certificate, caste certificate, income certificate and other applicable certificates as Physically Handicapped/Freedom Fighter/Military Personnel need to be uploaded in the required format in the application form of MP MBBS BDS 2018.

Step 5 – Payment of Application Fee
Candidates after uploading of documents, need to click on ‘Proceed to Payment for Registration’ after ensuring that all the details entered are correct. In case, the candidates need to make any correction in the MP MBB BDS application form 2018 then they can do so by clicking on the ‘Back to Registration Form’ button. The payment of application fee has to be made online after which the receipt of the online transaction for MP MBBS 2018 will be generated. Candidates can keep this receipt for future reference.
